Help Me Grow Kent Care Coordinator
Help Me Grow Kent (HMGK) is an outreach and navigation service that connects families with children ages 0-5 to existing resources within the community. HMGK also helps caregivers understand the development of their child through the Ages and Stages Questionnaire. These essential functions happen through telephone intake, incoming referrals from Partner Organizations, and through the HMGK Website. The Help Me Grow Kent (HMGK) Care Coordinator is responsible for acting as a point of contact for incoming clients, document referrals, provide timely follow-up for clients who have been referred for services, generate reports, and various other reasonable essential functions for HMGK.
